Permanent Supportive Housing offers rent subsidies and case management services for the following homeless Contra Costa residents: individuals and families that have a dual diagnosis of mental health and chronic substance abuse and individuals with chronic substance abuse problems. If you are receiving services from a county or nonprofit agency, please contact your worker for an expedited referral.
Homeless adults diagnosed with a mental illness live in community shared apartments and pay 30% of gross monthly income towards rent.
